you can create a page with a built in template which have a form hard coded in it.
then you can automatically create a page with the same template when ever you register a sub site.
its like creating another page similar to about page when ever a sub site is registered

I am also looking for this..

i have a 3 tier registration form which i have to add in wordpress..which will be a difficult task so i thought it will be easy to fill up the table which are populated when one register using wordpress registration.
Pleaselet me know the table and fields which get populated when one register using a wordpress…

I forgot to mention one important point
right now when someone click on category 1 i am displaying the post which is under category 1.i need to display the post under category one and also need to display a list kind of or you cna say display the thumbnail and Excerpts of all the post of child category

I am able to resolve the issue
i have made the wp-content folder permission as 755 and it works fine now
