The “Module Size” option needs to create QR Codes, that are the same size, throughout the whole website…
Done, user can either enter module size in pixels or overall image dimensions as [width]x[height]. Note that both width and height values must be equal.

The Widget UI should be more similar to the TinyMCE UI.
UI issue fixed by including editor.css for WordPress version 3.4.1. Not sure if works with every theme.
Issue has been fixed as promised.
Plugin version 1.4 offers a choice whether to preserve URL queries in permalink or to trim queries and make URL more simple and QR Code smaller. It is admin’s choice:

Sorry, there is a confusion in understanding what actually is a permalink. Some users recognize permalink as link to a specific page (applies only for pages) while some users count in also links to categories, tags, etc. But in general you are right, a “permalink” QR Code in widget should display the exact location where scanned.
This issue will be fixed in next version (1.4 and later)

The Widget UI should be more similar to the TinyMCE UI
We are trying to make plugin UI similar to WordPress UI, as generic as possible. Widget UI should be very similar to other widgets by design. A pop-up dialog in post editor should also be very similar to other dialogs, i.e. insert/edit link dialog. But we cannot guarantee consistency in design for other themes.
Thank you for all suggestions. We have upgraded the plugin – widget supports permalinks since version 1.3. TinyMCE feature is more complex to implemented so we’ve dropped it in this version.
