Which strings were localized and now are not? Are those strings part of the WP core, or from theme or plugins?

Indeed there is a conflict with the theme.

It may be a conflict with the theme. But can’t say for sure without having the link to the page that contains the form.

No. The form relies on Javascript which is not compatible with AMP.
