It seems you are reading the situation mostly right. I have created a video just to illustrate the issue and make it super clear. I would be happy to pay for the extra support just to finish this last part off if necessary. I need the solution urgently.

Showing in the front end is not the issue it works just fine. its displaying the information on the backend that I need. I just want to see the check boxes on the back end on the users profile that is all.

I read it and saw that I need to insert the %field_slug%. I am not sure how to do that do I place it in the HTML field block. Like so…

yeah I changed it now, look under gallery tab on the tabs on the left

added it to new website and it does not appear at all ? weird
