Thanks for the reply.
I’ve tried plugins deactivation. Revolution Slider is causing problem, but I am not getting th exact issue.
I’ve even tried removing the functions from plugin core files.

Actually You are not getting my point sir.

For the temporary solution i had to define the name field again in second form and hide it, Then [your-name] tag used in “from” field in “mail” tab.

Thats how it is made to work for now… but you have to suggest a genuine solution for that.

It’s not resolved yet.
I’ve followed your guide ..made the test. but the issue is still there.
On removing custom services it disappears and on adding services it appears again.

deactivated !!
Standard wp theme activated !!
