Apologies for delayed response. Agreed field labels ‘bug’ could have been the problem from what you describe.
I am training a volunteer on maintaining our rescue website and this includes building a mirror site from scratch on another server. Happy to take up your idea of similar/same site and drop in a couple of duplicated forms to see if values removed on duplication. Won’t be able to do this though for a couple of weeks – is that ok?

Hello Nithin
Thanks. Agreed that once ‘value’ re-instated all works fine.
In retyping values, like you, I found many missing but not all across our forms. Before I raised this issue ALL forms worked perfectly so why the loss of some values? Just for your information and consideration the only changes I made in Forminator before the forms failed on ckeckboxes was to duplicate one form as I wanted to keep the original but place a revised copy of it heavily amended for up front usage on a trial basis.
Could that for some resaon have created a general loss of value entries across our Forminator forms?
Hello Predrag
Still require help!! The issue was in hand with Adam who asked for code export – nothing heard from you until now and the problem is certainly NOT resolved nor temporary.
We use your forms extensively in our rescue and with lockdown easing in the UK we are struggling with rehoming, adoptions and volunteer application in particular. Tried you behaviour suggestions – no diffence either way.
Just completed an adoption test form online with two screen grabs of parts of form with lots of checkboxes and the resultant email received from the website – these illustrate the issue continues:
In case you ask the code export for this form is at https://pastebin.com/z6z3s81J
Grateful for early response.
PS – this issue only arose over the weekend just gone.
Hi Adam
Thanks for prompt reply.
Yes, checkboxes have always worked without issue since all forms created over a year ago.
Have exported one such form (as all with dropboxes affected in the same way). See https://pastebin.com/grQg6ye2

Okay it’s not 26 March yet (my last message above) but hey. yes, all now seems to be resolved through installing an SMTP Plugin.
Let’s call it resolved. Thank you.
Good day Jorge
Happy to go along with your suggestion.
There are 9 Forminator forms on our website and we receive 2 or 3 completed ones a day so a ‘wait and see’ approach may give results.
I have have put a review date in the diary for 26 March and will get back to your then.

Hello Jonathan S
Understood, sorry about no log. Have tested CCF Application form for two days now having installed and configured Easy WP SMTP plugin. No errors showing on website’s emails log, so that’s good news. As I said previously it was only that one form that had issues.
Happy to treat as ‘resolved’ albeit that there is a blip of sorts within the WP PHP handling now overridden by the SMTP solution.
Thank you all for your efforts.
Hello Jorge
Asked hosting company. Their reply was “Sorry no, the mail server logs are centralized are not per domain, and also they will have been rotated since January anyway (probably only going back a couple of weeks).” I had asked to go back to 28 Jan when this problem arose.
Will look at SMTP login though until the above date ALL Forminator forms worked without issue using the WP default for email despatch.
Does the absence of hosting company logs mean there can be no progress to resolution?
Best regards
Hi Nithin
Yes red Xs failed in all cases to reach target email addresses (3), all green ticks fine – some successes of the problem form, others different Forminator forms which have no problems. Cursor over red X reads “Could not instantiate mail function”.
This particular problem first arose on 28 Jan 21 with failed email as previously stated.

Hi Adam and thanks for prompt reply.
Your questions:
– fixed (3 addresses within the charity)
– not applicable
– not SMTP using WP default PHPReviewed form setup in Email Notifications Recipients Tag they are set at:
– Email Label etc – all OK as required
– Recipients – ‘Default’ 3 comma separated email addresses
– Advanced – no settings
– Conditions – ‘Send All’Last night and today test emails were once again completed and logged by the plugin you recommended (amongst other forms). Have updated form submission data and annotated it in relation to the log screen grab.
Really strange that some come through and others seem simply never to get on their way down the email ways! Does this help? I am copying form text from HTML in the plugin and forwarding manually to the three addresses where a form has failed to send as email.
Thank you. Prompt and excellent response as always!

Thanks for your response/message. Unfortunately family issues have kept me away from computer for some days!
Yes, the dev 14th line change does work but on reflection the complexity of finding/returning to the form via a browser (and with each having different routes to cookies) this particular solution is not really user friendly enough for ‘Joe Public’ completing a Formintor form on a ‘save and resume’ basis.
Perhaps one day your dev team can incorporate something with a simple ‘save pending send’ button. Nevertheless very grateful for your prompt response and my apologies for not fully understanding the save-in-cookies methodolgy until now and for not responding earlier.
Nastia
Why I know not – I did not insert it but above the macro {All_fields} a unidentified code line was in place – once removed all was ‘normal’.
A special thanks to you Nastia and your development team for resolving this protracted issue. Great team – great plug-in! We can get on with our charity work.
