When you tried it I had Jetpack deactivated. I just reactivated so please try it now.

I just removed the conflicting plugin hummingbird.

Same thing I am using. After clearing my cache/cookies the Add Media button is gone but adding a link does work. One problem I did notice is that once you hit Text, you’re unable to go back to Visual.

I’m not sure why but it’s still not working for me.

Update:

After deactivating Jetpack, the form did initially work (with the exception of the “Text” button) but now all the buttons that were originally not working that became fixed are back to not working.

Ok so I put that code on the designated page and put the shortcode within the form (using 123contactform) but the shortcode in the form still displays as just text

I also tried putting both

and

I would try contacting your hosting company. Worse comes to worse you can always upload themes in your FTP file manager.
