I would like to add a +1 to this.
It does not add an image, smiley or video to the Visual Text editor.

Versions:
Wordpress 3.5.1
bbPress 2.2.4
toolBar 0.75

Google Chrome.

Symptoms:
When I upload a file, I get the file upload processing dialogue.
When it gets to 100%, it then dislays

filename 0.2mb Failed.

Checking in wp-content/uploads, the file is there, and has been uploaded, but is not displayed in the editor.

Indeed I am! I’m not using the menus at all, I’m displaying the page structure. I’ve just added a menu.

Turns out I don’t want a menu. These aren’t the ‘Droids I’m looking for.

So… the question is, is there a plugin which displays the children of the current page as a list?
