Since this is a conditional form, I’ll have to add and style all of the fields. Will all of the fields show in the sent email even if they’re blank? I only want the filled in form fields to show in the email.

I figured I was missing something in the CSS. It works and displays perfectly now. I appreciate the quick and thorough response.

I wish, but unfortunately, I haven’t. I hadn’t received a response from the author of the plugin, so I just gave up and used a different plugin all together. CF-7 is a good plugin, but you’re pretty much on your own when troubleshooting.

I suggest Gravity Forms. Very inexpensive and excellent technical support.

I was aware that it was an HTML5 element. I know that I’ll have to add some javascript in order for it to work properly across browsers, but I have no idea what script and where to place it.

I have just downloaded your plugin and as of now I’m having difficulties using the wordpress thumbnail function. When I upload the image I select “thumbnail” but it still shows the full size picture. The link to the page on my site where your plugin is implemented is:
