Im having a similar problem, I updated and then it stoped working…. got the premium version.. asked for help on SupportDesk.. I will see what happens…
Then I found this post… same problem but different error in console:
TypeError: c.yithautocomplete is not a function
Any ideas? Different Problem??

Despues desde Admin/Woocomerce/Impuestos creo los distintos Tipos adicionales de impuestos: IGIC, IPSI Ceuta, IPSI Melilla .
Por ultimo creo 4 Tasas de impuestos segun Pais/Estado ( deben aparacerte las provincias de Espa?a cuando selecciones Spain como pais) :
1.- Spain > Ceuta
2.- Spain > Melilla
3.- Spain > Las Palmas
Spain > Sta. Cruz de Tenerife
4.- Spain > álava
Spain > Albacete
Spain > Alicante
Spain > León
Spain > Badajoz

Brevemente… La unica solucion que Yo he encontrado para solucionar esto es agregando las provincias de Espa?a y después crear Tasas de Impuesto paara Ceuta, Melila e Islas Canarias y una tasa para todas las demás….
En mi caso que tengo Woocomerce 1.6.1 creo, agrego las provincias en class-wc-countries.php, os las dejo abajo que es un co?azo hacerlas todas.
Si necesitais mas info desarrollamos el post…

Got It working adding code to function.php, but Id like to use a custom plugin better… So, if anyone can help me around with the Class not found issue…

Why on the page where Ive added the code
[pmpro_signup level="1" short="1" intro="0" button="Signup Now"]
I dont get the extra fields? Ive tried with short=”0″ intro=”0″ also…
I can only see the extra fields on the page I added the shortcode [pmpro_levels].
What Options does [pmpro_signup] have? How can i print them?
