The issue is not the plugin but in your WordPress theme or other 3rd party plugin that is overriding the style of the form. you need to to check with a developer to troubleshoot it.
Also we have refunded you already long time ago because we could not troubleshoot it because of the typhoon.

For the FREE version, other shipping method should be removed so that it will not show on cart or checkout shipping section.
But for the PRO version you can, with this method.

I think your using another plugin to trigger free shipping when the order is € 90 above. that is why it is not working.
We recommend to you to use one of our plugin “Innozilla Conditional Shipping & Payments”
here is a test we made:
scenario: when the order total reaches $300 or more it will trigger free shipping. if not it will trigger the per product shipping.
