Every label and value is sent except fieldname204 (checkbox). All other fields are text area fields or single line text fields. Fieldname204 is the only field that has dependencies and upon checked choice it is showing fieldname 121 or 122.

What could be the problem?

How do I make the input value bold? I tried with <%fieldname101 separator={{ }}<b>%></b> but it didn’t work.

Thank you for quick reply. Can I use the same code structure for changing input color and label color on all labels and inputs? For example:
