Unfortunately, i couldn’t identify a pattern and it happens to about 1/3 of the orders. Problematic orders can have any type of payment method, delivery method, variable or single products, etc.

I could send the url through email if that is possible.

I can confirm that all the above settings are as you suggest.
There is no caching on the site.
