This thread may be marked as resolved. The issue here was due to a bad IP address that had been entered and not a technical issue with the plugin. For others who may be experiencing a similar issue of your site being open, unrestricted to all–and whose issue is not due to a caching plugin conflict…I would suggest–as another step in you troubleshooting–going through your list of unrestricted IP addresses one at a time, testing the front end after each one to see if you find a bad one. If you have a short list, maybe even delete the plugin completely, then reinstall and add each IP address in to the unrestricted list and check the front end after each add. If you enter a bad IP address, it will immediately open the site to all visitors–then you’ll know which IP is causing the issue. Might not work for everyone, but this is what fixed a random case of the plugin not working for us.

Just checking in to see if anything made sense with the info I sent you. The site is still open with no restrictions and people are accessing what should be paid content. Do you see any settings here that would impact that functionality?

I have the site visibility set to restrict site access to logged in visitors.
The handle visitors setting is set to ‘show simple message.’ Then we have approx 48 IP’s listed with access to the site.
