Looks like there maybe a plugin conflict of some sort.
Few things to check first:
– Are you running the latest version of WooCommerce? Should be 3.0.3
– Are you running the latest version of WooCommerce Services? Should be 1.3.2
– Are you running the latest version of WordPress? Should be 4.7.3If all above are up to date, please test your site for theme and plugin conflicts.

The best way to determine if the issue is being caused by a theme and/or plugin is to do the following:
1. Temporarily switch your theme back to the Twenty Sixteen Theme:
https://www.ads-software.com/themes/twentysixteen/
2. Disable all plugins except for WooCommerce & WooCommerce Services.
3. Test to see if the issue has been resolved.If that resolves the issue, then slowly re-enable features until you find the one that’s causing the conflict. If it doesn’t resolve the issue, please let us know so that we can help you further.
Please note that switching to another theme will not delete data and you will be able to revert back to your original theme without any issue after testing is done. Your site will simply have a different display during the test.
Switching to a default theme and deactivating plugins is the first step in troubleshooting issues. That allows us to target it quickly.
