I haven’t been able to recreate this problem (yet).

Be sure that you’re not currently logged into WordPress as an administrator when trying to test, your IP address is not in the list of active Unrestricted IP addresses and that any access keys that you may have used on your computer are not currently activated.

There was a bug in the code to create the IP table. It has been fixed and a new version of the plugin (1.2) has been posted to the WordPress plugin repository.

Please deactivate and delete your copy of the plugin then re-install and activate the new version to clear up the issue.
