Customers were all added manually. There is no form on the site.
This is a brand new (child) multisite.
Yes I customized the status names using the same instructions you referenced.

Contact Statuses: Lead,Proposal,Design,Development,Customer,Refused,Blacklisted,Cancelled by Customer,Cancelled by Us (Pre-Quote),Cancelled by Us (Post-Quote)

Funnel Statuses: Lead,Proposal,Design,Development,Customer

Please note: that I just have only 3 contacts (all are customers) in the system at this point and for some reason they show up in each layer of the funnel.
