This plugin automatically starts working once activated. There will be no icons in any corner of the site to indicate that.
When the ‘Tota11y’ option is checked, there will be an icon shown in the bottom corner on the front-end of the site. This is not needed for the plugin to work, but simply an included tool to help you see any additional accessibility issues with your site. You can learn more about that tool here: https://khan.github.io/tota11y/
Color contrast & font size issues are not fixed by the plugin. That is up to the site owner to correct on their own using the Divi options.

This is an error within the latest version of the Tota11y tool that Divi Accessibility has included. Because this is an issue within the 3rd party tool, we are unfortunately not able to resolve this error message.
I am not sure why that would be saying that because there is clearly descriptive text within that link so I would ignore that message. If you would like you can report this issue to the Tota11y tool itself by visiting https://github.com/Khan/tota11y

In reply to: [Disable Responsive Images] Is it still needed with 4.6.1?@mikeverk That is dependent on if the issues you had before were fixed in the themes/plugins that were causing them. You can deactivate the plugin temporarily and check for any issues. If anything does appear just reactivate it. ??
I have tested this plugin on 4.6.1 and it is still working with removing the responsive images introduced in WordPress 4.4.
