“A while” depends on the values you entered in the cache page.
What values did you insert into the cache page?
Is WP Cron active on your site?

The UsersWP plugin does not interfere with gravatar resolution. (Tested).
I even liked the plugin and I’m going to use it.

In the beginning it is normal to continue to present the gravatars of gravatar.com. Because the cached gravatars are rendered in the background by WP Cron.
Give the plugin some time to process the gravatars. If some time has passed and the plugin continues to display gravatar.com from gravatar.com. See if you have WP Cron active on your site.
However I believe it is the UsersWP plugin that is interfering.
I do not know the UsersWP plugin I will analyze.

Adding support for CDN is not at all my idea, it would be almost impossible for me to support all existing CDNs.

Very well.
When I have time I will try to create an environment identical to yours so that I can understand what is causing this problem.Until then the plugin is left without W3TC support.
These synchronization issues will happen even with version 2.
If the images are not synced to CDN nothing is done.

Does the “Service Worker” of your site interfere by caching old gravatar?
Do you confirm that the images have not been synced to the CDN?
If you want, I can help you test the “Service worker”, just change your gravatar again.

In version 2 the only difference in Lazy load is the support for 2x.
The option to choose whether the plugin will use lazy load or not will be retained.
Allowing to apply classes that can be used by themes or plugins to identify the gravatars.
The use of LQPI will not be for version 2. But I will analyze later.
It will keep the option that allows to use an image or without image for lazy load. But only if the lazy load of the plugin is used.
As in version 1.4.3.
