Thank you for sharing those links. The issue is coming from a CSS rule of the Elementor plugin.
Could you add the following CSS snippet on Formidable > General Settings > Custom CSS?.with_frm_style .frm_top_container .frm_primary_label { text-decoration: none !important; }
