Yes, the [auto_id] shortcode is only available in the Pro version. You could also avoid the error by making sure the entries are not identical when submitted.

The error usually appears when submitting entries with identical values.
You can use the auto-increment shortcode to assign a unique number to a hidden field in each form entry, and this will make each form submission unique, bypassing this issue. However, this is a PRO feature, and it will require the Pro plugin to be enabled and active.

I’ve seen this behavior before, usually, is related to a setting on a SMTP plugin. Could you confirm if you are using a SMTP plugin for email deliver on your website? If so, could you share which one?

Thank you for the screenshot. To confirm the site is running the latest Formidable Forms version? The field doesn’t look like our standard and current Checkbox and Radio field options.
