I want to be able to ask if the participant is willing to provide demographics information then if yes show 4 additional questions (Gender, Age, Education, Ethnicity).

Thank you for your response that was exactly what I needed and it is now working as expected. In the end, I added to following code to my customized form-login.php template file:

I hope that was an accurate solution.
