I have a custom template, I have a variable for date/time ready to go. How do I populate a specific text field before the field elements are populated on the page?

It is a simple page that contains just a short-code, no custom templates. A correction to my original post: it only happens on a page with [pdb_record], and not on a page with the initial [pdb_signup].

This is what it’s generating:

Could any of the settings cause this to happen? It’s pretty much straight out of the box. I haven’t changed much.
