Thank you so much for your reply, unfortunately it didn’t work for me, so I did a rollback of the NinjaForms plugin and now it works again. I’ll wait for the next update, if it’s not working I may consider trying another forms plugin.

Thanks for your quick reply! You’re right, I used the Health Check & Troubleshoot but didn’t switch themes.

When I convert back to a standard theme the issue is gone and the form appears. However, when I change back to my theme (Coaching) and enable all plugins the form disappears but when I only disable Yoast, it appears again.

So, I’m not sure what my next steps are ??

I have the same issue but with the date range field.

Even in the first field, everything I select gives the error: This is not a valid date value.

Additionally, I can’t go to future months or years in the first date field.

(FYI I’m trying to use this field so people can request to book a Bed & Breakfast for a certain period)
