It was only that specific bit of code that was pasted into the box. But no problems! I deactivated and then reactivated the plug-in and that solved the issue.

I’m having a similar problem and your solution makes sense, but whenever I try to copy the correct form action URL into the box on the admin page nothing happens when I click save changes. Nothing has changed when I open a new tab and look at the page with the opt in form on my site. Whenever I try to refresh the admin page the following code is pasted into the box:

Thanks for your reply. I updated my original post to reflect that my homepage is a static page. I will make a similar post on the support page for my theme but if anyone else has information please feel free to post it here ??
