I have the same issues, followed the steps you recommended and this didn’t fix it. My WP install is on my local server and not hosted remotely.

It was a fresh WP 3 install. No upgrade. I tried to disable it many times. Removed the files, reinstalled, etc… and it didn’t fix it.

I’m going to try it again today on another new site. With a fresh install. I’ll let you know if it happens again and the steps to reproduce.

Check the permissions on your files (Chmod)
