I gave it one more try and found my own mistake. Still a great plug-in. Sorry for my bad!

What is there not to understand how your plug-in works?
Yours is not the only plug-in I tried for that matter.
Besides, I’ve had already said it nicely that I used your plug-in earlier once,
before you stripped it down to this version I tried now.
Obviously you push your pro version, which is understandable for the sake of money.
But don’t try to belittle my knowledge and review, and rethink about your attitude towards users instead!

It works very well.
First: You should read what the author wrote, what to do, to work with it!
Second: You should have a bit decency and change your poorly review!
Thanks to the author for this merely 6kb neat plugin!

I tried some other working plugIns inbetween and solved my problem.

This is correct. The same result by me. You can not use “Form Actions” tab in WP 4.5.

Tested with WordPress 4.5. What a pity!!! “Form Action”-tab in WordPress 4.5 not working, which means, one can not enter Email Notification “Send to email(s)” “Send to responder”. And so, it is quasi useless for WP 4.5.
