I’m sorry but this is not possible. The renew orders are created a day before the renewal date and the status is on-hold.
In this way, the order cannot be paid manually on the My Account page. And this avoids that the order being paid twice: manually and automatically in PayPal.
When PayPal send the IPN the status change automatically from on-hold to complete.

I’ve made a test in my local installation and all works fine with guest checkout.
Try to do one of these:
1) check the sandbox API credentials
2) make sure that you are testing with a sandbox account user
3) try with the live credentials sometimes PayPal sandbox has problems
