OK…not mine, but must have been developed by someone who developed the web site. I have listed all my Plugins, so it must be custom code then. Been working fine for years though…only just noticed it is not working. WordPress 5.7 has just been installed on my site, so maybe that’s affecting it???

I have quite a few of these questionnaires I have copied and alteed the text to suit different training courses I run. The one above is for using Outlook Effectively.

Sorry Takayuki, I might be misunderstanding your question? The form has built in CSS/Code I’m guessing to calculate the scores for each question.

Still having this problem…. really keen for someone to point me in the right direction to solve this. Geoff

I’m still trying to work out exactly what happens. I’m not sure my users get an error message.

When I test it, the Submit Button Greys out…if you press it again, it goes Green. (I understand this might be called the Spinner…?)
Yesterday I discovered the initial time you press it, the form goes into Spam…the second time you press it, it is successful.

I can see this in Flamingo… before I installed that form, I never understood that.

I have the same issue. I have some Questionnaires in COntact Form 7 which will not work via Internet Explorer…
