Thanks for getting back to me. It was is already checked. So the problem is that the placeholder does not show, which means that the visitor wouldn’t know what to put in the fields. When I wrote first time the fields did not appear at all, they do now but without text.
Where it says “Input Field Replacement Text” do I need to write it in a certain way?

Hi, thanks for your response. I have not published anything yet so I can not give you an url where you can see an example. Anyways, after googling it seems that it has to do with the “Caption alignment justify”. I will need to use left alignment instead.
