Success! Finally!
I deactivated Wordfence, dropped the two security tables manually, and reactivated Wordfence. Updated settings to require 2FA for administrators, enabled the grace period for activating 2FA, and enabled XML-RPC (because I use Jetpack). Saved settings. Settings saved. Activated 2FA for my admin account and signed out. Signed back in, OTC was required for sign in. Upon login, went to Login Settings > Settings and saw that 1 administrator (me) has 2FA enabled.
Problem solved. The information about the security tables was the information I needed to fix the problem. Thank you for your help.

I’m not sure. Pretty sure I used to see a complete list of IPs that were blocked, but there is only one from a couple of weeks ago.
I’ll try dropping the security tables manually and let you know how that works.

Perhaps deactivating Wordfence, dropping those two tables manually, and reactivating the plugin will work?

I tried your latest suggestion, but it didn’t work. When I selected the checkbox to delete those tables and saved the settings, it was unchecked after the page reloaded. Still, I went ahead and tried activating 2FA, but it was not activated.

Thank you for your reply. I tried @senjowo’s suggestion even though my issue is different. It didn’t fix my issue.
I sent the diagnostic report as requested a few minutes ago.

I was just going to let you know that it is Gutenberg that’s the culprit. I was creating my first product in WooCommerce, which I just installed the other day and noticed that the Age Gate checkbox was there. WooCommerce doesn’t use Gutenberg (yet).
It’s getting late so I’ll download your current build tomorrow, unless you release it before I get to it.

Here’s a link to the screenshot you requested. I marked it up to show you about where I expect the Age Gate checkbox to be.
Do note, I have Gutenberg Demo plugin installed so my screenshot isn’t exactly the same as the one you posted, but it’s the same place. Perhaps Age Gate needs to be updated for Gutenberg?

In reply to: [Age Gate] Selected ContentHi, I seem to be having the same issue. The checkbox to select specific content to age gate is not appearing in the Publish section. I’ve tried it with Posts, Pages, and Projects. I’ve tried with existing drafts and new documents. It never shows up.
I tried repeatedly saving the Restrictions and Access settings,r as you suggested above, to no avail.
I haven’t yet gone through the process of deactivating plugins to see if there are any conflicts. It’s an option though. Still, that’s something I normally do only when there are reported errors.
Can you provide any additional suggestions?

I deleted the appointment that had the odd phone number, which I had previously deleted from the record. There weren’t any suspicious characters in any of the other fields that I recall. But, deleting that record fixed the issue.

I don’t recall doing anything out of the ordinary in the days leading up to experiencing the problem… I created some draft posts/pages, updated some plugins. That’s pretty much it.
I did some additional testing. I deactivated all of the plugins I’m using. Then I reactivated only Ultimate Appointment Scheduling. The 500 error appeared.
Next I deactivated Ultimate Appointment Scheduling and reactivated all of the other plugins I’m using and there was no 500 error.
Next, I remembered that I added an appointment in which the phone number was a Russian number. I thought maybe that the non standard US phone number format (even though I’m pretty sure there is no format for the phone number) or the + for the country code was causing the problem, so I edited the appointment, directly in the database, and removed the phone number. 500 error remained.
I think there is one more thing I need to test… trying a different theme, like Twenty Seventeen. I’ll do that at my earliest convenience and report back what I find.
But I hope you’ll look into this soon. I currently have the Ultimate Appointments plugin deactivated so that I can access the admin area of my site. But I’d really like to get this plugin turned back on soon.

I hadn’t added or modified any appointments when the error seems to have occurred. But on a whim, I went and looked at the appointments table in the database. I had inadvertently entered a phone number in the email field. I deleted it, enabled the plugin and it’s all good now.
Perhaps there should be a check to ensure, if anything is entered into the email field, it is a valid email address format?
