[UPDATE] I followed Matthias’ recommendation and switched over to Explicit Mode. I temporarily enabled Direct Analysis Mode and Analysis, then submitted a form. I chose the related form event in the Analytic Box to have enhanced spam checking. Then I disabled Direct Analysis Mode and Analysis.
So now, in Explicit Mode, the plugin is checking for spam form submissions using Thrive Architect forms and isn’t blocking ManageWP access.

Well, the problem is back again. I’ve tried PHO v8.1, 8.2 and 8.3. I’ve whitelisted the event type. But it’s still not allowing access by ManageWP. When I deactivate the GDPR plugin, ManageWP access is restored.
Disregard. I noticed that the site was using PHP v7.4. I updated it to the current one, and the issue disappeared.

Not sure what you mean by “system account”. But, all sites are self-hosted on various shared hosting accounts. None are WordPress-hosted.

Each of the options you mentioned have been checked for a long time.
None of the usernames are the same as the display names.
Had another login attempt today on another site using the actual backup admin cryptic username. So far, the hackers are only seeing the *backup* usernames.
