Solved the problem by activating (turning on) the CAPTCHA setting…….Go to the “settings” page for this WordPress plugin to make the change.

I have a similar problem……when an email is submitted from the WordPress Form, the email recipient (in this case myself) always received two duplicate copies of the email. I am using WordPress 3.4.2. This is most annoying.

Love your EMAIL PETITIONS PLUGIN. However, there is a problem with the petition overlay (when using the petition as a widget) if the theme is TwentyEleven. That theme uses a rotating image banner at top. If one uses the Email Petition Widget with this Theme, clicking the “Sign Petition” correctly opens your petition overlay, but the theme’s banner image remains on top — and the petition overlay is hidden below the image (not viewable).
