The issue seemed resolved, but: after 2 days without problem the issue spontaneously reappeared. When copying the site to a development version on the same server the problem did not occur.

When selecting a service, the loading icon keeps running, and in console there is the message:

I tried again to downgrade to 5.0.28, but this time the problem was still there. Only when downgrading to 5.0.26 the function recovered.

After downgrading to 5.0.28 and update again to 5.0 29 the described issue did not occur anymore. So the problem is gone without explanation and all is well.

Problem solved. Last issue was caused by hidden second instance of the form. Great service, thank you.

Thank you. This makes a difference: now the booking form is shown correctly. But: when I try t pick a date and select one from the calendar, an error occurs, and the waiting icon keeps spinning: “An unexpected error occoured.”

Hello John,
When you inspect source code of the page you probably will see the complete date. I guess the sidebar column is too narrow to show the complete table cell.
