It seems your Facebook application id is wrong.
You can use 503595753002055 for testing purpose.I have tested with this app id and https://www.facebook.com/wesley.media.arts in my website and its working fine.
Just replace your FB application id with mine (503595753002055), it will work 100%.
Test it and let me know..

https://www.facebook.com/sean.michael.imler. This is not facebook page, It is Facebook Profile.
You need to add valid facebook page to work it . Example is https://www.facebook.com/WordPress

I saw your website & it seems its working fine in inner pages.
I think on home page, issue is with theme, my widget may conflict with your theme. Though i am not sure..
