I have exactly the same problem, with post metadata missing on “category with header” pages (not sure about other page templates, as not putting posts on any other type of page).
WordPress 4.2.2, Nirvana v 1.0.6
From looking at the HTML, the “entry-meta” DIV element after the post title on the category page is empty, whereas on the individual post pages, it contains a <SPAN> element containing sub-elements that contain the metadata.
Any suggestions?
