– Since I have been using CF7 on hundreds of website, I would like to understand too: if looking at a front-end page can help you debug the issue, could you please tell me what kind of checks can you do from a browser?

– I consider the error reported as a bug, indipendent from the single website: there is no kind of admin notice telling the website administrator or the compiling user that a file was attached and not received, and it could be discovered only by looking at PHP logs.

BTW, by any chance do you happen to know if it is possible to clear the opcache in a multi-instance environment like Docker Swarm / Kubeneretes?
I realized that when the PHP-FPM is replicated clearing the cache means clearing it only on the single instance/pod… I can’t find much documentation on the issue, but maybe that’s because PHP CMS like WP are not that popular in multi-instance environments!

When using hashtags, it works. With username (exactly the same of the previous server – where it was doing fine) it does not.

Is that PHP fatal error the likely cause?

Apparently, tables was urrecoverable due to MySQL problems happened during Wordfence scans.
This is related to the infinite/continuous Wordfence scans problem that I had last year.

The solution was to export all database except Wordfence tables,
drop the entire database and recreate it,
import Wordfence backup,
and start from zero with Wordfence activation.

I know about the 500/1day option,
but I would like to “turn off” this option to fasten things,
also because I would like to avoid Wordfence to reverse-DNS or lookup-IP to make my websites fast.

As @jackarray suggested, I want to use Worfence only for manual scans and password hardening / email when login.
All the rest, we do with other server-side software, and we want our websites to be fast.

I suggest you to give the possibility to turn off what you call “firewall” but leaving options inside “firewall page” still usable in next versions.
