My problem was also my fault, I did not realize that UM does not recognize metadata shortcode.

So I changed the title from custom metadata {teamname} to {display_name} and it is working nicely.

thank you for your response. Unfortuantely, I was not able to find it anywhere.

It is not in Fields Manager (when I try to add new field to form).
It is not anywhere on the form settings page.

So where is it? Can you send me a screenshot?

I know it is annoying, but I was trying really hard to find it.
