https://codex.www.ads-software.com/Changing_File_PermissionsAre you using any customization in your theme’s functions.php file? If yes can you test by using a fresh copy of original functions file for your theme? Or test with a default theme?
Check your .htaccess file, back it up, then delete it and then test your site.

May be its not hardcoded into theme. First check your site’s widgets and menus. If you don’t find it in a widget or in Menus then look into your theme.
Go to Appearance » Editor. You will see a list of files on your right hand. Click each one of them, press CTRL+F and paste this text string you are searching for.
If that does not work out. Connect to your website using an FTP client, like Filezilla. Download your theme directory to your computer. It is possible that your theme developer may have also created a child theme so download that too. Now you can opoen each file in these theme folders in a text editor like notepad. Or you can use Agent Ransack recommended earlier by robthirlby.

I had this code installed in my site specific plugin which automatically removed image links when I add images to post. However, I expected SEO slides to work like WordPress posts and add an image whether or not it is linked to media file.
