$orig_full_path = $upload_base_dir . "/" . $original_sub_path;
to this:
$orig_full_path = $original_sub_path;

It looks like it could be a path/directory issue.

Ok, disabling all the plugins didn’t work either.
I did disable some, but I will go back and disable all of them to see if that makes a difference.
