Hey @sweeppicker – Thanks for following on this ticket, and I apologize for the delay due to the weekend.
I inspected the form, and when I tried to submit the form, I noticed “The form was unable to submit. Please contact the site administrator”.
To fix the issue, please try one of the options below:
1) Set a site cache lifetime of around 45 hours, and this should be sufficient to avoid submission errors.
2) You could change the cache lifetime of the WPForms form token, and for this, please check out?this article in the WPForms developer docs. Please note that this requires editing your website’s code, and is recommended for developers.
3) Disable the anti-spam feature in your form. If this works, please enable another anti-spam option for your forms.

I tried visiting the URL that you’ve shared, however, I’m seeing page not found error message. If you’d like to exclude specific fields from email notification, and considering that you’re re-creating the form in WPForms, you can remove
{all_fields}
and use the{field_id="#"}
smart where you can pick and choose which field should be included in email notifications.However, in case you’re using the plugin that you’ve mentioned, I’d suggest you to please reachout to the plugin developer, and they should be able to assist you.

I looked at the screenshot, and the settings looks correct. Unfortunately, we’re unable to replicate the issue on our end.
In this case, when you get a chance, can you please replace the field smart tag in the Reply-To field with a hard-coded email address? Once you’ve added the email address, could you please let me know if the issue is still there?

I imported the form to my dev setup, and on my end, the multiple choice with image choices appears to be working fine. In this specific case, I’d suggest you to please try testing for plugin conflict and see if that helps.
