I’m having this same issue, I generated the form again but it is still not working.
I had to integrate the code by using the shortcode integration for php:
<?php echo do_shortcode(‘[mc4wp_form]’); ?>
The form field and the button appears and gets formatted perfectly but I can’t send the form. Once I click the button nothing happens.
