I tried to add registration form within client portal page itself. This led to the only form showing (screenshot).

Does it mean that a new page needs to be created for register?

Yes, it worked.

Another issue is that once the form is submitted, it is showing thank you message. But the form keeps displaying (screenshot). It will be better if form no more remains displayed.
