what you are asking is not possible in this version but i will add that functionality in another update.

Yesterday i have submitted the plugin on the codecaynon. It is still in review. When the plugin gets selected I will tell you.

Yes there is a reply button but it is available on the premium version.

The css class name will be applied on the input tag. I checked it … it is working fine

Do the following things ….

1. Use this shortcode [wpad-comment-form id=”YOUR_COMMENT_FORM_ID”] where you want to display the comment form.
2. Where you want to put the shortcode disable the comment form from backend. If there are multiple comment forms then there will be conflict.

The issue you are having is not from this plugin. I checked the plugin and it is working fine. May be from some other plugin it is causing ??
