I cleared the hosting cache and was then able to access the plugins page. I deleted your plugin and replaced it with another. I’ve had no issues since.
To answer your questions, I had no other redirection plugin at the time of the problem. The redirect I tried to set up was a simple 301, where I had to change a URL slug. I was just looking to redirect from the URL with the original slug, to the URL with the new slug.
