Yeah, just add text and a title above it.
Leave the title and the text empty to remove it from the widget.

I’d assume the issue here is with your host not allowing mail to be sent via PHP. Try using WP Mail SMTP (find the plugin in the Plugin Directory) to send emails.
You can get details on how to send emails from your host.

This is because once an email is confirmed, you cannot confirm it again.
The “redirect to a separate page on error” is a feature in the premium version.

I’d recommend that you ask on your signup page for users to check their spam folder. Sometimes if your admin email is @gmail.com, there can be issues. I recommend changing it to @domain.com (where the domain.com is the same as your domain name where WordPress is hosted).

If you place padding-top:10px; in a CSS rule on #wp_email_capture & #wp_email_capture_2, you will add 10 pixel padding to the top.

I see you’ve bought the Premium version (thank you!), and a feature you have access to is the autoresponder with it.
Alternatively, on the landing page, try placing a “Please bookmark this page for future reference”.

No they cannot. The reason being is that by and large unsubscriptions are handled by the mailing list providers, rather than the plugin.

I have just signed up successfully.
The temporary emails won’t be causing an issue (I’ve had successful signups from sites with temporary emails running into the thousands). I think what is happening in your instance is people are clicking on the link to view the PDF and then clicking on it again at a later date.
My advice is rather than link to the PDF, link to a page on the site with a link to the PDF. State on that page to bookmark this page if you wish to access it again.
