Disabling plug-ins didn’t make any difference. But disabling Automatically add Paragraphs fixed the problem.
Why didn’t disabling Automatically add Paragraphs help when I tried it earlier? It seems that was because I had two widgets containing the Ninja form on the same page, as I showed you earlier. As I mentioned then, the first widget was an ordinary text widget and the second was a SO editor widget. The form worked correctly in the first but not in the second, even when both had Automatically add Paragraphs disabled. But simply deleting the first widget caused the second widget to start working correctly! (This is what you can now see at https://staging2.richardwein.com/)

1. I’ve switched from my child theme to the standard Sydney them, to get rid of a possible source of complications. So don’t be surprised that the page looks very different now.

3. I wasn’t sure if you wanted the new page to have the form shortcode in an SO Editor widget, so I’ve made two:

I hope you don’t mind me continuing to comment, but I thought this might be helpful as I seem to have had a similar problem to down2be’s.
I just recreated my problem by adding back a Siteorigin Editor widget (with the Ninja Forms shortcode) to my page. I’m using a staging site at staging2.richardwein.com. It has restricted access, so you have to login with username and password both “-123”. If you scroll down to the bottom, you can see a working form (using an ordinary text widget), followed by a non-working form (using a Siteorigin Editor widget with the same Ninja Forms shortcode). I’ve just cleared the Automatically add Paragraphs box, and it didn’t make any difference.
Let me know if there’s anything else you’d like me to try.

I had a Ninja Forms shortcode in a Siteorigin Editor widget within a Pagebuilder page. It worked correctly until the Siteorigin Widgets bundle was updated (a few weeks ago I think). After the update all I got was a rotating circle where the form should have been. I solved the problem by replacing the Siteorigin Editor widget with an ordinary WP text widget.
