ISSUE FOUND: Ok, I found the problem and Yoast has nothing to do with it. I should have guessed.

if you are using… WP 4.0 > Genesis Framework > Yoast SEO

Do not install the following plugin

Top 10 Popular Post Plugin

It will effectively disable Yoast SEO

Same here. I have no idea why.

I’m also having a problem with the system thinking that the photo alt tags are missing when in reality they are there.

There are a few other funky issue too.
