First of all, what theme do you use? Did you try to create the widget with standard WordPress theme. Maybe the problem is in the theme.
Secondly, what type of widget you inserted? Didn’t your forget to click “Save” button (I know, ridiculous, but sometimes people forgot about it)
How does your widget look on the back-end?

So, is this like “Print” button? User choose the location and then click “Print” and get the map on the paper?
Did you try to use the search for Google Maps plugins and try some of them?

Where do you find it? I checked your site briefly and couldn’t see it.
Does the word “popup-pomo.php” sometimes add to the site address in the address bar?
Try to check your permalinks settings in the WordPress (Dashboard-Settings-Permalinks)

You can try to disable all your plugins one by one and check this issue after every plugins deactivation.

Usually 500 error means that you have Fatal Error on your server.
In this case you need to enable debug mode and you will see the text of the error. With this information you can either fix it by yourself or contact, e.g., theme or plugin developers if the problem is in the theme/plugin.
