The issue has been resolved, thank you for pointing out what the real problem was and I have referred it to my host for support and they said that it was the problem related to my theme not with the plugin (duplicate post).

im ruuning WordPress 4.6.1 and the plugin version is 3.0.3

Does it happen only in grid view or also in the list view?

i think it wont matter because the images were offline when i clicked the thumbnails, it seems like the images were never uploaded.

Does it happen again if you reactivate my plugin?
